SUPPORTIVE CASE MANAGEMENT SERVICES
Supportive Case Management assists individuals
in recovery seeking additional supports. Staff work with individuals to attain and maintain recovery and make plans for achieving goals.
Services include: helping to maintain housing, access to community support services, and obtaining referrals to substance use treatment programs.
Program requirements: Individual is in
recovery and has stable housing.
(There is no housing component to this program)
